On Monday, August 30th, 2021, we said a final goodbye to a remarkable man who touched many lives. After a short but valiant battle with Creutzfeldt-Jakob (Prion) Disease, Darryl passed in Shelley’s arms surrounded by the love of his family. Darryl began his career in the elevator industry as a summer job while studying engineering, and never looked back. He started as a helper, got his ticket, and later opened the operations for Fujitec Canada in Alberta in 1999. Under his guidance, the Calgary operation proved to be the most successful branch in North America every year until his retirement at the age of 68 in 2017. He was a mentor to many, and a trusted voice who valued honesty and hard work. Darryl was always a fit and active guy. In the winter, he spent his leisure time skiing, vacationing, and windsurfing with his family in many exotic places. Summers always included an annual trip to the Gorge with the windsurfing gang and weekends at the cabin in Sylvan Lake. A favourite was a yearly trip to Cabarete, Dominican Republic with Jill and family and his many friends. Windsurfing, beach walking, family adventures and happy hour were always high on the fun scale. After retirement, together with Shelley, he undertook a huge renovation at their Sylvan Lake cabin. First built by his family in 1962, the cabin was very close to his heart. With hard work and grit, Shelley and Darryl upgraded their special place these past few years to be their forever home. His ingenuity, creativity and attention to detail can be seen in every corner of his most rewarding project. You can feel the love he put into the cabin - and his presence is felt everywhere. Until his passing, it had become their happy place, sharing summers with family and friends. Darryl’s love for Shelley was profound. Their love story is admired by all who had the privilege to know them. Darryl never failed to tell her how much she meant to him. They were best friends, and he was her rock. He wore the joy and pride of his children, their spouses, and his grandchildren on his sleeve. His love of wind sports, tinkering in the garage, creative problem solving and zest for life were just a few of the many valuable lessons he passed on to his grandkids. He will be forever missed by his family, his windsurfing gang, lake neighbours, work colleagues and longtime friends. For Darryl, a great day was one spent on the water, hanging with family and friends, enjoying a black russian - or a margarita - and laughs in the hot tub. He will be lovingly missed by his wife of almost 51 years, his children Matt (Alison), and Jill (Ben), three grandsons, Evan (9), Levi (13), and Mateo (10). In lieu of flowers or gifts, donations may be made in tribute to Darryl’s life. The Alberta Prion Research Institute has collaborated with the Alzheimer’s Society of Alberta & Northwest Territories to support research on Prion Disease. You may make your donation directly to Prion Disease research in the notes section of the donation page, at tinyurl.com/darrylcervi As per Darryl’s wishes, there will not be a formal funeral, but the family intends to arrange a celebration of his life at a later date when restrictions ease. My Tribute Gift Foundation makes this online charitable giving portal web page MyTributeGift.org (the “Website”) available to you subject to your acceptance of and compliance with these terms of use (the “Terms of Use”). Your donation will be processed through My Tribute Gift Foundation for distribution to your selected charity. Your credit card transaction record will appear as My Tribute Gift Foundation. An official tax receipt for your donation will be issued by My Tribute Gift Foundation. Donations are sent out to charities at the end of each month for all donations received in that previous month. So a donation made on the first day of the month would be sent at the same time as one made on last day. Payments would be sent either by paper check or direct deposit. Please note that the there is 3% + 35¢/transaction fee for credit card processing, My Tribute Gift Foundation collects a 5% administrative fee. These fees help fund the cost of processing all donations, issuing tax receipts, and sending all appropriate information to the selected charity. These fees are assumed by your selected charity, but you will receive a receipt for the full amount of your donation. There are NO additional costs the charity has to endure to participate and accept donations through MyTributeGift.org. According to the American Institute for Philanthropy (charitywatch.org) a charitable organization is considered “Good” if they are spending less than 30% on administration and fundraising costs. My Tribute Gift Foundation helps reduce those administrative costs for charities and allows donors to contribute to lasting memorials for their loved ones.
When donating online, if the donation form has an option to add a tip, there is no convenience fee. The tip is optional and can be adjusted before you submit your donation. The amount of the tip will be displayed to you before you confirm the charge to your payment card. Tips are in addition to the donation amount, so they do not affect the amount that goes to the charitable organization. Under IRS and CRA rules, tips are not tax deductible to the donor.
You warrant and represent that all information and materials you provide to My Tribute Gift Foundation, through the Website or otherwise, are accurate and truthful and do not violate third party rights. Indemnity. YOU AGREE TO INDEMNIFY AND HOLD HARMLESS THE FOUNDATION PARTIES FROM AND AGAINST ANY AND ALL CLAIMS, DEMANDS, ACTIONS, SUITS AND PROCEEDINGS BROUGHT BY YOU OR ANY THIRD PARTY (COLLECTIVELY "CLAIMS") ARISING OUT OF YOUR USE OF THIS SITE IN BREACH OF THESE TERMS OF USE OR THE INFRINGEMENT BY YOU OF ANY INTELLECTUAL PROPERTY RIGHTS OF ANY PERSON OR ENTITY, INCLUDING ALL LOSSES, LIABILITIES, DAMAGES, COSTS AND EXPENSES (INCLUDING ALL LEGAL FEES) ARISING FROM SUCH CLAIMS.
If in any part of these Terms of Use is held, by a court of competent jurisdiction, to be invalid or unenforceable, such part of these Terms of Use shall be eliminated to the extent permitted in that jurisdiction, for purposes of that jurisdiction, and the remaining Terms of Use shall otherwise remain in full force and effect.
These Terms of Use and your use of the Website are governed by the laws of the Province of Ontario and all federal laws of Canada applicable therein. The courts of the Province of Ontario shall have exclusive jurisdiction over any dispute arising out of these Terms of Use or your use of the Website.
These Terms of Use constitute the entire agreement between you and My Tribute Gift Foundation with respect to the Website, except as we may explicitly agree otherwise in writing.. No delay or failure to exercise any right with respect to these Terms of Use shall be construed as a waiver by My Tribute Gift Foundation.
As a donor, you are responsible for researching the charity, understanding how your money will be used, and understanding the tax implications of your donation. We will email you a receipt, but the deductibility of your donation depends on your individual tax situation. We are not financial, tax, or legal experts. Please do not rely on our information to make decisions about your finances, taxes or legal matters. Consult a qualified professional in your jurisdiction.
Participating organizations have chosen to actively participate in the “Sponsor Program” to be represented in the Sponsor Charity array in an effort to increase their exposure and the opportunity for more memorial donations to be made to their organization. A rate structure is agreed upon and assessed on each donation processed through that program according to each organization’s individual agreement. Giving through that program has helped many organizations realize a drastic and significant increase in net donations.
If a donation form has a line item of Convenience Fee. The Convenience fee is displayed to you prior to approving the charge to your payment card. The convenience fee is in addition to the donation amount, so they do not affect the amount reaching the charitable organization. For example, a $100 donation becomes a $101.50 transaction. The convenience fee is directed immediately to a for-profit service provider. According to IRS and CRA rules the tax deductible receipt will therefore only be issued for the subtotal amount.
My Tribute Gift Foundation seeks to distribute all funds to organizations provided they have maintained active 501c3 certification in the United States or registered charity status in Canada. MTG relies on the beneficiary organizations to maintain and keep up to date contact information on file with the IRS or CRA respectively. This must be done in order for My Tribute Gift Foundation to exercise care and reasonable efforts to distribute payment promptly, without undue cost and burden to the foundation. In the event that a payment has not been deposited 12 months from issuance, the foundation reserves the right to utilize those funds in its mission to the foundation.
My Tribute Gift Foundation reserves the right to modify, change, discontinue and suspend these Terms of Use, the Website, and/or any services offered through the Website, in whole or in part, at any time, without prior notice to you. Your continued access or use of the Website after modifications to these Terms of Use have been posted on the Website shall indicate your acceptance of and agreement to be bound by these Terms of Use as modified. It is your responsibility to review these Terms of Use regularly.
